HealthReach Community Health Centers

HealthReach Community Health Centers provides family medical care with integrated behavioral health at 11 medically underserved locations in central and western Maine. We are Federally Qualified Health Centers and serve people of all ages and backgrounds offering sliding fee services based on family size and income to the uninsured and underinsured. All of our sites are National Committee for Quality Assurance Patient-Centered Medical Home certified.
